TRAINING ISSUES
Documenting CCU and ITU experience It is essential that trainees document their CCU and ITU experience. This is best done by keeping a summary log of the cases seen on CCU and ITU and linking it with reflection or assessment. This should then be signed off by your Educational Supervisor to be of any use at the G(I)M PYAs.
MRCP Diabetes & Endocrinology This exam has to be completed and passed by all trainees appointed after August 2007 before their PYA. We recommend sitting it ASAP and well before your PYA.
The Kelly-Young MRCP Diabetes & Endocrinology Prize This prize is awarded annually at NERRAG to the youngest in terms of training year StR passing the MRCP Diabetes & endocrinology exam. Richard Quinton secures the funding of £400 and it is named after 2 distinguished former Endocrinologists in the region, Bill Kelly and Eric Young. We are pleased to announce that Murali Ganguri won the prize this year. Well done.
Critical incident/complaint If you are involved in a critical incident or if reporting an incident concerning training issues please inform your supervisor and the TPD. Ensure they are reflected upon in your portfolio
Portfolio Completion It is essential for trainees to engage with their portfolio on a regular basis and record learning. It is also essential to record the numbers of patients seen as news or reviews for clinics, on-call, ambulatory care. It is essential to record the number of specialty clinics undertaken. Undertaking this activity means that your Educational Supervisor should be able to engage with the portfolio so as to provide you that assessment for ARCP purposes.

Thanks to the collaborative work by the trainees and trainers in the region, our programme is progressing well. All the units have their StR slots filled this year!
The Northern Endocrine network is developing well. Anna Mitchell presented the extensive region wide audit at NERRAG with 6 trusts participating. The Website (www.endocrinenortheast.co.uk  )  is live now and the team headed by Praveen Partha is working on strengthening it. The link to the endodiabology blogspot also will be placed in that site. Earn Gan is working on developing a research day  A group of trainers and trainees headed by Ali Aldibiat and Anna Mitchell are working on developing modules in diabetes and endocrinology for post graduate certificate in research and we have plans to identify ways to fund our StRs to get this training. We had a new Trainee representative elected by balloting by the StR. Congratulations Ali Aldibiat!
The GMC survey showed we were in top 2 in 3 domains and within top 4 in a total of 7 domains. We aspire to be number 1. We shall continue to work towards achieving excellence! Once again, Thanks for all your hard work.

NEERAG-Shaz Wahid
NEERAG this year was held in September due to the domino effect of the American Endo and BES moves. Turn-out was good at 50 people and the speakers were excellent with plenty of interactive discussion. Clearly as word gets out that NERRAG has moved to end of September/early October more will come. Spread the word. We shall circulate the date for 2016 soon.

NEW GUIDELINES FOR THYROID EYE DISEASE-Petros Perros

It is estimated that there are about 2,500 new cases of TED every year in the UK.  The figures probably have declined over the past 10-20 years for reasons that are unclear. Nonetheless far too often TED is undiagnosed for several months, or misdiagnosed as allergy or conjunctivitis adding to the frustration of patients and denying them treatments that can make a difference. Simple, safe and low cost interventions early on can make a big difference. Selenium supplements, lubricant eye drops, prisms, meticulous attention to controlling the thyroid, avoidance of radioactive iodine and smoking cessation, can prevent deterioration of the eye disease and improve quality of life. In 2009 the European Group On Graves’ Orbitopathy (EUGOGO) spearheaded an international drive to improve the care of patients with TED. The Amsterdam Declaration for people with TED was signed by 84 national and international professional and patient-led organisations including the BTA, pledging to improve patient care. In the UK TEAMeD was born (Thyroid Eye Disease Amsterdam Declaration Implementation Group, www.btf-thyroid.org/index.php/campaigns/teamed) aiming to implement these objectives in the UK. In particular diagnosing TED early and getting patients to be seen in specialist centres was a high priority. And one way of achieving this is clinical guidelines. TEAMeD embarked on this project three years ago and was able to secure support by the Royal College of Physicians as part of the College’s “Concise Guidance” series.  After extensive deliberations between experts in endocrinology and ophthalmology and expert patients the guidelines were published this year (1). They focus on four key areas: (a) criteria for early diagnosis of TED by GPs and endocrinologists (b) initiating simple, effective treatments as soon as the diagnosis is made (c) guidance on which patients should be referred to specialist centres and (d) empowering patients with Graves’ disease to recognise the early symptoms of TED.  

It is hoped that the guidelines will facilitate early diagnosis and better treatment of patients with this disfiguring disease.
